MINOR PASSENGER CONSENT,

RISK ACKNOWLEDGMENT & EMERGENCY AUTHORIZATION

1. AUTHORIZATION TO PARTICIPATE

I authorize the minor identified above to participate in a recreational fishing and/or sightseeing charter operated by Sound Bite Charters Ltd. in the coastal waters of British Columbia.

I understand that participation may include vessel travel, recreational fishing, handling fishing equipment under crew supervision, viewing marine wildlife, and boarding and disembarking.

I acknowledge that fishing is optional and that the captain may restrict a minor's participation in particular activities for safety reasons.

2. ACKNOWLEDGMENT OF INHERENT RISKS

I understand that recreational boating and fishing involve inherent risks, including:

Slippery decks, docks, and boarding surfaces.
Unexpected vessel movement, waves, wakes, and changing sea conditions.
Falling overboard, cold-water immersion, hypothermia, and drowning.
Injuries involving fishing hooks, lines, knives, and other equipment.
Injuries involving fish or marine wildlife.
Seasickness, dehydration, and exposure to weather.
Mechanical emergencies, collisions, and delayed emergency response.
Serious injury, permanent disability, or death.

I understand that reasonable safety precautions cannot eliminate every inherent risk.

I have considered the minor's age, maturity, abilities, and individual needs before authorizing participation.

3. SUPERVISION AND SAFETY REQUIREMENTS

I agree that the minor must:

Attend and follow the pre-departure safety briefing.
Follow all reasonable instructions issued by the captain and crew.
Wear a properly fitted lifejacket or personal flotation device whenever required by law or the captain.
Remain within authorized passenger areas.
Avoid operating fishing or vessel equipment without crew authorization.
Refrain from unsafe or disruptive behaviour.
Notify an adult or crew member immediately if injured, frightened, or experiencing difficulty.

I understand that the captain and crew are responsible for their applicable operational and safety duties.

Where the minor is accompanied by a parent, guardian, or designated responsible adult, that adult is expected to provide appropriate supervision without interfering with the captain's authority.

If I will not personally accompany the minor, I authorize the responsible adult identified above to accompany and supervise the minor during the charter.

Sound Bite Charters Ltd. may decline to carry an unaccompanied minor or require additional supervision where reasonably necessary.

4. MEDICAL INFORMATION AND EMERGENCY AUTHORIZATION

Required emergency medication or medical equipment, if applicable:

I authorize the captain and crew to seek emergency medical assistance for the minor when reasonably necessary.

If I cannot be reached promptly, I authorize the accompanying responsible adult to communicate relevant medical information and assist with emergency arrangements.

I understand that emergency treatment will be provided or authorized in accordance with applicable law, including any legally required consent from the minor or another authorized decision-maker.

This authorization does not override the minor's legal rights concerning consent to medical treatment.

5. FISHING LICENCES AND REGULATORY COMPLIANCE

I understand that a minor participating in recreational fishing in British Columbia tidal waters must have the appropriate valid fishing licence and comply with applicable Fisheries and Oceans Canada regulations.

I acknowledge that a salmon conservation stamp may be required when retaining salmon, and that catch limits, area restrictions, closures, and recording requirements apply.

I agree to ensure the minor has the necessary licence before fishing.

I understand that Sound Bite Charters Ltd. cannot guarantee the retention of any particular fish.

6. LEGAL RIGHTS AND LIMITATIONS

I acknowledge the inherent risks of the minor's participation and authorize their participation subject to applicable safety requirements.

This form does not waive, release, or extinguish the minor's independent legal rights or claims.

Nothing in this agreement excludes or limits any liability that cannot lawfully be excluded under the Marine Liability Act, applicable Canadian maritime law, or other applicable legislation.

This form is intended to document informed parental or guardian authorization and relevant safety information, not to transfer the Operator's statutory responsibilities to a parent or guardian.

7. PRIVACY

Information collected through this form will be used for passenger administration, safety, emergency response, and other lawful operational purposes.

Medical information will be accessed only by persons with a legitimate operational or emergency need and handled in accordance with applicable privacy legislation.

Promotional photography or video use requires separate consent.

8. PARENT / GUARDIAN DECLARATION

By signing, I confirm that:

I am legally authorized to consent to the minor's participation.
I have read and understood this agreement.
I understand the inherent risks of recreational boating and fishing.
I have disclosed relevant safety or emergency information.
I understand the captain's authority over vessel operations.
I authorize the minor's participation.
I understand this document does not waive the minor's independent legal rights.
I am signing voluntarily.
